My impression is the only OS where this is a problem is macOS.  And if
it has a packaging system then groff isn't in it.  So instead of
FreeBSD's ‘install package groff’ prompt when mandoc has trouble, Apple
plough on and use mandoc.

The one escape Apple do offer is their man program checks for groff and
uses that in preference to mandoc.  So nmh can either recommend groff be
obtained on macOS or if nmh is packaged in something like Homebrew then
that's the point where a ‘depends’ or ‘recommends’ could be used.
