I agree with Gould 100%:
    1. Without a "tied-to" accidental, "starting at the beginning of the 
system" (e.g.) will almost certainly lead to confusion or errors; and,
    2. Once there is that initial accidental, the rest of the measure should 
act according to whatever accidental rule is in effect.

That being said, giving Lilypond users the option of (e.g.) changing the 
tied-to accidental to/from a cautionary is not a horrible idea, in the interest 
of flexibility and choice.
