In fact, this was due to unescaping the '. If I amend your Dutch entry
to:
%A Johan \(cqt\~Hart
%D 2003
%T The Title
%Z Hart
and have in my document:
.R1
database t.ref
no-search-ignore
no-discard
abbreviate A
reverse A
sort Z1A1
.R2
then everything works perfectly. The entry appears in the references as:
't Hart, J., The Title etc.
and is alphabetized under 'Hart'.
