I'll leave to other people the divination games, without being able to
reproduce the problem there's no way for me to grasp what's going on.

That said, there are two things that can be said wrt. ratpoison "meta"
and firefox:
- ratpoison "meta" works by sending fake key events to the underlying
  application. Some applications ignore those keys (eg. xterm has
  a setting for this).
- ratpoison had a bug in its "meta" implementation earlier; firefox
  tended to ignore the faked key, because it received junk instead of
  a proper key event (IIRC the most important problem was that firefox
  didn't like the timestamp).  This problem has since been fixed.  Maybe
  the way we create those faked key events isn't perfect yet?
