That's an example of someone teaching LilyPond to produce forms of
notation that are very different from the default output.
I love Karlin's idea of pointing you to the Clairnote project. That has templates of everything you need. He creates his own noteheads, and modifies the staff. I think that starting out with his code, you could make the changes rather quickly.
