Wow, that's an old version.  The utility convert-ly is for updating old 
files but it will almost certainly *not* work on such an old file 
without further manual editing.  I don't know why you can't get the 
printable version so the only option I see is to try to convert the file 
and then see what errors you get and try to make the appropriate changes.

Maybe someone else has a better idea for you.
