Maybe this interferes with the definitions in common-macros.itexi
that you already mentioned in your patch...
Nope, this can be reproduced with pure texinfo.

Here are the current diffs of the pdf manuals.
(git texinfo.tex vs. texinfo.tex from the Lilypond repo)
Green old, red new.
They consist mainly of the non-indented @verbatim blocks now.
