It looks like something in the macros printing the
page header/footer is causing a break.
There's generally two ways to try to avoid this:
1. use the no-break control character (default: "'")
instead of "." to introduce requests in the page
header/footer macros, or
2. switch to a different environment while processing
the page header/footer.
