Three that I use (all GPLed):

(i) Emacs with the appropriate addon 
modes (psgml, nxml) is good for certain xml editing
(text-heavy), though it can IMHO be a bit of a drag for 
structure-heavy (this is just down to the modes in question,
not emacs really)

(ii) xmlcopyeditor is GPLed, and though is also oriented
towards text (editing large volimes of "copy"), its tag-folding is
particularly  nice, making it IMHO pleasant for structure-heavy
editing too:

(iii) KDE Kate has a nice enough tag-folding XML mode and
xslt highlighting. (though you can't seem to fold tags in xslt
mode, which is a bit strange. I haven't tried the latest Kate,
but it is presumably as good or better).
