documentation errors

I get the following Octave documentation errors when doing
make_index in octave-forge:

isunix: No matching address@hidden deftypefn'.
struct: No matching address@hidden deftypefn'.
debug_symtab_lookups: No matching address@hidden defvr'.
dassl
 stdin:62: address@hidden' expected `ifinfo', but saw `example'.
 stdin:96: address@hidden' expected `example', but saw `deftypefn'.
 stdin:56: No matching address@hidden example'.
 stdin:7: No matching address@hidden deftypefn'.

Attached patches contain changelog entries.

I'm not an expert on texinfo, so check the dassl changes
carefully. I put @iftex around the @tex regions, but I don't
know if they are required --- the html builds fine, but that
uses @info instead of @tex.
