I do it manually, using openjade and pdfjadetex.

IMHO, it's a very stupid idea to check xml process instruction in the
prolog to see whether the document is XML or SGML, because it's not
strictly necessary to have one.  DTD is enough.

Though we can't do anything about it, Debian's tools are in the wild.
Also adding the <?xml ?> process instruction will have cause harm, so
I'm adding that now.
