The Debian bug report resulted in a suggestion that solved the
problems.  Just add <?xml version='1.0'?> to the top of the .xml file.
See Debian bug #224110 for the whole story.  I think texinfo should do
this.  The seems much easier than generating both 3.1 and 4.2.
