Thanks Konstantin for the description.
Concerning the first/last name problem for the index, this is a problem
common to all languages, that cannot be solved at the MapOSMatic level
(without having a *huge* list of prefixes which would never be up-to-date).
We're thinking of asking the OSM project about this requirement and see if
something is doable at the OSM database level.
