    - Shift some material into the lead sentence.
    - <hy>: Document request default in lead paragraph.
    - <hy>: Drop redundant statement of inapplicability of placement
      restrictions to exceptions defined with .hw.
    - <hpf>: Tighten wording and consolidate paragraphs.
