old phone list file format?

hi all,

this week my SIM card was ruined and I lost the phone list on it :-(
I have a old backup file from xgnokii with the list but
apparently the file format changed and xgnokii doesn't read the file
correctly. 

I looked around the file formats and apparently there are 2 new fields
before each number in an entry (except the first). Can anyone tell me
what they are and how I can replicate them to make the list I have a
valid one?
