tracing of FT2

Dear friends,


ftview now has two options, -d and -l, to activate debugging
resp. setting the trace level.

Since FT2 is still beta, I've defined the FT_DEBUG_xxx macros by
default.  Additionally, I've changed some trace levels to make the
output more reasonable -- level 0 is very sparse, but at level 7 you
are flooded :-)

A lot of tracing code is missing IMHO.  Suggestions for improvements
are highly welcome.
