The reference to "also use at least version 6.7 of
texindex and of makeinfo from the texinfo-6.7
distribution" sent me down a rabbit hole, trying
to replace only texindex, texindex.awk, makeinfo,
and texi2any with those from texinfo-6.7.
Installing texinfo-6.7 from source made it all
better.

Thanks also to Antonio Giovanni Colombo, who
explained that the Italian version of the
texinfo.tex file was not the problem.
