I think I found the problem.

In the affected file I found a single closing double quote (i.e. no
opening quote): ``. After I removed it I reactivated
font-latex-do-multi-line and edited the file again---the slow down
was gone. But I can not reproduce it with a sample file. So I used
your code to scramble the file---but some macros were affected, too.
I'll send it to you in a private mail.
