FWIW:

IANAL, but from what i understand (from the sqlite and Fossil projects), to be legally valid such a waiver has to be sent in physical form to the license holder/manager, and that person is responsible for ensuring the physical safety of those documents. e.g. Richard Hipp (sqlite/Fossil) keeps his in a fire safe.
