Setting PROMPT_COMMAND='echo -ne "\033]0;address@hidden"' works outside screen, I was wondering if it possible to change it dynamically.
Make screen change the Xterm title to the current screen title as I switch screen windows, Ctrl –a “ or Ctrl – a a or Ctrl – a 1

I am not quite sure what you need but, if you just want to change the window title of putty you can set the variable PROMPT_COMMAND as you wish.
PROMPT_COMMAND='echo -ne "\033]0;new_title\007"'
