The copyright notice it's the line with the '(C)' inside, you can get
information about it here:

   http://www.gnu.org/prep/maintain/html_node/Copyright-Notices.html

About what names you will put on this line, consider that this person
or company will have all the rights over  the source code, so I think
that you may want to put your name at this line.

That you put on your message it's the license notice, this must be go
under the copyright notice.
