Ouch, that wording terrible. Sorry about that, can't have been easy to
follow...

A lot of my responses would be theories, but they all contain: broken bits in
the database, not enough error recovery in the backend scripts, and bugs that
have crept in over the last ~6 months.
We/I will be studying a lot of the code history in the coming weeks (if it
takes that long). A definitive answer will come soon; we're (re-)developing
the solutions pretty much every day; all critical fixes are in now, though.
