[Qemu-devel] some fixes for monitor

Hi,

I have some small fixes for the monitor, mainly regarding the virtual
console version of it, which is the default now. In the order of
appearance:

- fix crash when the guest OS set a smaller size, and then monitor is
  activated.
- do not advertise ioport logging if support is not compiled in.
- add an "all" target for "-d".
- output "info pci" to the console.
- add a term_fprintf, which redirects stdout to the console.
- fix endless loop in "info history".
- output "info registers" to the console.

BTW Ctrl+Alt+digit is fantastic. Thank you!
