The error indicates a problem with the usersfile, are you sure you have
it installed as /etc/users.oath and it is readable by the PAM module?

But I still suspect your real problem is that TOTP is not supported.
Well, it is in 1.10.0 which is going to be released within minutes.  So
please upgrade. :-)
