Reproduced:

Redhat 7.3, no software raid.
/usr (must be separate partition, works fine if /usr is on /)
nss-mysql 0.43

lsof on /usr right before the umount request:

starting to look more and more like a RedHat bug, although
it's very hard to reproduce on other systems (RedHat included).
