Yeah, it's certainly something you don't want to hard code since the
majority of my application/octet-stream attachments are either PDF files
or Microsoft Office documents, not text.

In MH-E, I use C-u <attachment number> K e RET <program> RET to view a
particular attachment. That would indicate that you might want to be
able to enter the program as you're reading the email. That might be
easier than specifying the program for the particular attachment at the
command line.
