[Mingw-cross-env-list] [PATCH] New package: dcmtk

Hi!

This is a Makefile for dcmtk, a library that implements large parts of
the DICOM standard, in particular the DICOM file format for all sorts of
medical image data.

This package requires two tiny patches. The first patch is submitted
upstream: <http://forum.dcmtk.org/viewtopic.php?t=2563>. The second
patch fixes an issue that is already fixed in the upstream repository
(in a slightly different way):
