I'll be coming out with an updated version of the patch this week, taking the comments into account. Some work issues have had me sidetracked and left with less time to spend on the project than I'd like to have had. Nevertheless, I'm confident I'll be able to put the updated patch together.

It'll move the measuring to a method. Before drawing the index, all category headers will be measured, witth the font size changed for the headers as a whole if need be.

Possibly, depending on how that looks, a follow-up patch would add an ocitysmap option --header={wrap,condense} to either wrap long headers to the next line, or scale all of them if a single one gets to be too wide.

I'm open to suggestions on that.
