First of all thanks to Nelson Henry, Klaus and Foteos for explaining
things so well. I have added an entry to the mailcap file 

        image/x-xbitmap; xv %s ; ; test=test -n "$DISPLAY"

and this works. But the mystery is not resolved entirely since the
entry just above this reads

        image/*; xv %si ; ; test=test -n "$DISPLAY"

Still a bit mystified...

Oh, yes:

Lynx version 2.6 on Solaris 2.4 (the use of the word "spawn" is a
holdover from those bad old VMS days). All the comments are regarding
"http" and not local files. The server at the other end is 
Netscape-Communications/1.1 which sets the Content-Type to
image/x-xbitmap whether the file is gzipped or not.
