Follow-up Comment #15:

Done:
- the heap can be relocated by defining LWIP_RAM_HEAP_POINTER as a pointer to
the memory's address;
- both the heap and the memp pools can be relocated by including an 'extern'
definition (including the corresponding relocation attributes or pragmas) in
cc.h
