The workaround with manual page breaks works for now, but I fear the day
when I need to add a few measures or change the staff size or something like
that. 

It seems like a huge drawback giving up automatic page breaks for something
as basic as metronome and rehearsal marks in between staffs. After all it
does not seem to be the preferred way for page breaking. Should I file a bug
report? I could try to isolate the problem better once I have more time.

I use ragged-bottom option for the vocal score. When Lilypond produces a
half-full page, the ragged-bottom option stretches the systems to an
unnatural extend or leaves huge spaces between systems, so for me it is a
spacing issue caused by bad page breaks, which in turn are caused by bad
extent-estimate. But I guess you can see it both ways. I'm sorry if I
confused you with the title.
