The trick is to include each \header field within the corresponding
\score block, i.e.
\score{
 The music
 \header{...}
}

and then add

\paper {
 scoreTitleMarkup = \bookTitleMarkup
}

to tell LilyPond to typeset the title of each separate score
the same way as it sets the title of the full book by default.
