For example the GIMP manual is translated using gettext.

a POT per chapter, a PO per chapter/language

The workflow in GNOME is:
 log in
 book (reservate?) a PO
 download latest PO for your language
 translate strings marked untranslated or fuzzy
 upload the file via web (this closes reservation)
