Why bother with the overhead?  First, we only use the fact that a
mach_port_t is an integer internally; this will not change.  But if
you really want to think of this pair of integers as one, then:

  effective_handle_id = server_communication_threadid
                        << (sizeof (handle_id) * 8) + handle_id
