cvs remove -f <filename>

should do the trick for you.

This one I'm not sure I understand. What is it you need that

cvs up -CdP -r NEW_BRANCH_NAME

does not get you?

You might be able to do this with 'cvs history', but you'll have to do 
some massaging of the output. The docs show the data that can be
