    Any kind of explanations for above question will be really appreciated.

I think the answer to all the questions is the same: look into
texi2html, instead of makeinfo.  It has far more extensive facilities
for customizing the HTML output.

The next release of Texinfo (no ETA, but not any time soon) will use
texi2html.
