1. Since you are using IMAP, some of the groups (folders) that you see
   are coming from the server. Junk is probably one such folder on your account. This is created by the server.

2. Those messages which are marked by server as spam are
   going into Junk before reaching Gnus. Hence the split is /not/
   working for them.

3. Those messages which escape the outlook spam filter are going through
   your split chain and ending up in mail.misc.
