I have followed those very same instructions several times over the last number 
of months. 
1) is unlikely to be a problem since I installed 21.1.1 over a clean NT setup.  
Unless it means I 
should delete the vc-hooks.el that comes with 21.1.1???
What version is your Clearcase? I have 4.1 loaded at the moment.
Do you use CYGWIN or the native shell? 
I have even tried Xemacs - sacrilege I know 8-)
