Great ! Thank you ! Very important to see different clinical record examples (obviously anonymized ) so we cover most of the information. 

The sample about Tuberculosis is very timely, since we're developing the MDG6 module, specific for TB, Malaria and HIV/AIDS !

Current Treatment/Medication

Provisional Diagnosis: I think this will be same as Present Illness

Prognosis/Extra Info/Advise: That can be be put in Summary

Yes. Most information will be there. Tonight or tomorrow I'll send to the list a sample PDF generated with GNU Health latest development version, so we can work together on it !
 
Curious: Do know that Patient Evaluation part of the application is changing.  Is Patient Evaluation under Reporting  working for 1.4.4 ?  It wasn't doing anything in my local installation. Tried to create a new record but didn't work.

Patient evaluation should work in 1.4.4. We're working on the form, optimizing the data input process and the report. We should have 1.4.6 by next Sunday.

Spot on. Yesterday in my discussion with one of the senior doctors, this is exactly what we had discussed. He had pointed out that  it was difficult to find clinical history of a patient with current health reporting structure. He is going to define a basic case history of a patient .
Perfect ! Samples are very important . We are working - slowly - on having a sample / demo database. The Clinical History for each encounter should look something like this. For each encounter we would have something like 

- Header : Date, Patient, Referral
- Main Complaint
- Present Illness
- Past History : Illnesses / Surgeries / Medications / Vaccinations / Screenings / Obstetrics....
- Family History / Genetics
- Socieconomics / Lifestyle
- Review of Systems // Examination
- Summary

GNU Health have all the information already, so we just need to place it on the report
 
Would you like to review it as an example ?  Any idea when will the clinical history part of application be available ? I have to learn and improve my Python skills and can then be of some help to you guys.

Any idea is more than welcome ! Please send it !

We'll work towards a "standard" basic report. Then, each health center will have its own, as part of the localization process.

We were exchanging thoughts with Chris and Andrew about improving the usability of the patient evaluation process and there, we talked about the reporting of the clinical history. 

We'll be also making changes on the signs and symptoms. Yesterday, we added the Present Illness  in the main form.

1) I added a new report record from Administration/Actions/Reports
2) As the report didn't show up on Patient  (tryton client view)
3) i added two entries in health_report.xml
4) I went for upgrade option for health module in order for changes to take effect.
5) The new report option showed up on Patient (tryton client view) but ...

Question:

 Now it is showing 2 entries of the same type in  Administration/Actions/Reports ?  Did i do something wrong ?

I tried to mimic entries of other reports. I didn't understand why there is a Keyword "Print Form" in all three reports Medications, Vaccinations and Diseases and model value is Pediatric Symptoms Checklist Evaluation. Is it of any use ? 

The best way is to declare them in the XML files. Take a look at the current reports in the "health" module. That should give you some tips on how to declare the report and their links.

Take a look a the current default branch, where we're making the changes for the next stable release. We are working heavily on this section since yesterday. I think that the main / basic clinical history report should be within the "health" module. Although a more specialized reporting would need a different module .
