You can create the insurance companies either from the party menu or form the Health Configuration menu.

In this menu, you can also create the insurance plan and assign it to the patient.

Remember that the plan is a product, so that means that you can can assign a price and invoice.
Thanks a lot for the description. However, I have not figured out how we can configure a plan. What I expect from an insurance "plan" is a way to specify how the plan affects users costs. For example, a plan can have a list of hospitals services and medicines and specify how much of the costs is paid by the insurance company and how much of it should be paid by the patient. And therefore, when we assign an insurance plan (and the corresponding insurance company) to a patient, patient costs (and also the money which should by paid by insurance company) is automatically computed based on the medicines and services that the patient have used. Unfortunately, I can't find out how much of the process is currently implemented in GNU Health and how to use it. Would you please explain a bit more about the way insurance works in GNU Health?
