[Health-dev] Record duplicated in table gnuhealth_medicament_category

Hi All. Creation of unique contraint in table gnuhealth_medicament_category using columns name and parent fails.

There are two records (ids 87 and 89) with the same name (Antithrombotic medicines) and parent (79).

Record with id 89 (id = em_12_6 in file medicament_category.xml) corresponds to name Lipid-lowering agents.

Record whit id 90 (id = em_12_7 in file medicament_category.xml) do not exists in WHO List (April 2015).

After update name for record id 89 and deletion of record id 90, creation of unique constraint is possible.

Note: There are not medicaments associated to medicament category with id = 90.
