This seems to work:-

.sp 2i
.PS 18i 5i
for j=1 to 18 do
ellipse "Deri";

And run it with this:-

groff -Tpdf -p -P-p5i,20i pic.trf > pic.pdf

The two numbers after .PS are wildth and height, and after -P-p are length and 
width of the papersize.

If I run pic on its own against the pic.trf file, the .PS line is changed to:-

.PS 0.400i 18.000i 18i 5i

I think the first two numbers are now the actual dimensions of the picture 
created. I hope this helps.
