I missed that!

If you are viewing the pdf in firefox’s internal pdf viewer you might not have 
seen the 
overview panel, which does contain links to the different indexes. You may need 
to click on the 
button at the top left followed by the second button in the row of buttons 
which appear after 
you have clicked the top left.
