[Groff] cover sheet macros for mm

Morning all.

We have been using a set of mm macros from the 1980's (from SoftQuad I
think) as the cover sheet macros suited our test report format.

We need to change our cover sheets, and I thought I might as well bite the
bullet and use the new   .COVER/.COVEND stuff with the mm macros which come
with groff (previously mgm macros).

I had hoped to find an appropriate starting point  for a definition file
in /usr/share/groff/1.20.1/tmac/mm but the cover sheets are very basic.

Has anyone re-created something like the original AT&T cover sheets that I
could use as a starting point ?

Any help appreciated.
