[Groff] How to include a letterhead

I'm trying to create a pdf with full page letterhead. When I do:-

cat <<EOF >header.groff
.fam C
.in 0
.ll 21c
.lt 0
.na
.nf
.nh
.pl 29.71c
.po 0
.ps 11
.tl
.vs 12.7p
.PSPIC -I 0 letterhead.eps
.sp -29.71c
EOF
cat header.groff file | \
groff -P-pA4 | \
ps2pdf -sPAPERSIZE=a4 - test.pdf

the following happens:-

1. The first page is blank
2. The letterhead appears on the second with a blank line at the top
and the bottom is chopped off
3. All the text appears on the 3rd page

How do I get it be all on the one page?
