    [ms]: Document \n[DD] as a Berkeley extension.
    
    It appears in 4.2BSD's ms(7) (1983), but not in Lesk 1978.
    
    Also relocate the stuff about it also applying to tbl, eqn, and pic to
    the section on using preprocessors.
