[ms]: Fix documentation nits.
* doc/groff.texi (Indented regions in ms): Use less casual wording.
* doc/ms.ms (Headings): Use "Input" and "Result" as column headings in
code examples, not "Code" and "Output", for consistency with other
examples.
(Indented regions): Increase column separation to 8n in example for
better readability.
