Hm, well, the situation is this:

GNUmed checks the number of paperwork templates immediately
before and immediately after the upgrade from v18 to v19. It
expects 1 more template in v19 than it had in v18 - because
it adds one.

In your case it finds MORE than one template added to v19
which is neigh impossible from what I can see. That's why
it throws up its arms in despair.

Now, do you happen to have special SQL code being run
locally on the upgrade which just might insert another
document template ?

If not I can't tell.

I would recommend removing the templates check from
the upgrade .conf and re-running the v18-v19 upgrade.
If that succeeds (that's very likely) I would assume
your database to be in good shape and suitable for
1.4 client work.
