I would create a new patient
Lastname: Doe
Firstname: Family
encounter type: group visit
Subjective:
Attendants: Joe, Jane, Laura
Objective:
...
A hook script might propagate something appropriate into
each of Joe's, Jane's, and Laura's individual charts.
