No. The plugins send a message to the menu system which
triggers inclusion of a menu item for them.

Not sure I understand.

Not currently but this can be done. It only adds virtual
security, however.

Neither. It is way to rarely used to warrant a shortcut.
Alt-X is one of the shortcuts typically used for exiting an
application.

Those items which are under "GNUmed / Configuration"
configures *how* GNUmed behaves.
What is found under "Manage master data" doesn't (directly)
define the how - it rather manages the data itself.
